A rack shelf accident

The accident happened in one of the companies in our city. The company had been running a huge server room in which there were three large cabinets and a high rack shelf containing heavy equipment. The incident took place on the first of August this year. The staff were working in the office when they heard a dull sound. Two men left their desks and headed for the server room situated on the second floor of the building. It took them some time to open the door and enter the room, but when they were finally inside, they saw that the biggest rack shelf had collapsed. Of course, the equipment fell down to the floor, too, and it was no longer working. All of the cables and connections were know in a big knot, and lots of other details and elements were scattered on the floor. Unfortunately, they were unable to fix that all at once, and they didn"t have all necessary skills, so some other employees, who were not working on that particular day, were summoned to six the rack shelf and the computers. Of course, the most important question was what caused the accident. It turned out that the shelf was burdened with too much computers and that it was not attached to the wall, as it should be. But nobody knew who should be held responsible for that.
